Dr. Hoffman is a credentialed provider with Blue Cross Blue Shield (BCBS). Please contact your insurance provider before scheduling an appointment to verify the details of your specific plan and mental health benefits. She cannot determine if or what you will be reimbursed for psychological services.

Dr. Hoffman also accepts self-pay patients. Upon request, you will be provided a complete itemized statement at the time of service that contains all necessary information needed so that you can bill your out-of-network insurance. Please be prepared to pay for your session fee in full at each appointment.

 

Cancellations of appointments without a 24-hour notice and no-shows of appointments will be charged the full session rate.
